Common questions for agencies about our referral process, documentation requirements, and emergency placements

DBH agencies and authorized coordinators can refer participants by calling our intake line at 202-878-9161 or faxing referral documentation to 202-217-3775. We provide rapid response coordination for urgent placement needs and can often accommodate same-day admissions.
Referrals
We offer rapid response intake services and can often accommodate urgent placements within 24 hours. Our intake coordination line (202-878-9161) operates during business hours with on-call support for emergency situations requiring immediate residential placement.
Emergency Placements
Referrals require two essential documents: MH CRF (Mental Health Certificate of Residential Facility form) and LOCC (Level of Care Certification). Additional documentation should include clinical assessment, current treatment plan, medication list, behavioral health history, and any special accommodation needs. Insurance verification is also required for admission.
Referrals
Authorized DBH agencies, clinical coordinators, case managers, discharge planners, and behavioral health providers can make referrals. Individual self-referrals must be coordinated through an authorized DBH agency or provider.

Emergency placements include discharge from psychiatric hospitals, crisis situations requiring immediate stabilization, homelessness with behavioral health needs, or situations where individuals are at risk without immediate supportive housing intervention.
